Design with defective resolution settings
Some designers do not test or do not care about being compatible with most screen resolutions. They believe if they can show it to the business owner on a specific size screen the business owner will accept it as working. Although the business owner may be losing customers because they simply can not see the web page correctly.

Navigation not standard
Whether you choose to have top, left or right navigation menu, keep the nav menu the same across your website.

Many web sites have text over pictures which at times looks very nice. Research shows the functionality of doing this is bad for your visitors.

Home is a great place to be for most people but the word HOME does not belong in your title. Get it out. Make the title meaningful for the content that is on your page. Unless your home page is about home - then it is okay.

A simple often overlooked design function. It has nothing to do with the look and appeal of your web site. You should ask yourself why do I have my web site on the internet? The answer should be "to be found." If someone can not find you they will never do business with you. Same goes for the search engines - if they can not find you will never have any new business.
Pictures and Images using numbers
Image search is second in the type of searches people do. If you desire increased traffic on your web site get rid of the DSC000253.jpg. Name your pictures and images.
Pictures and Images without Alt Tags and Title Tags
Alternate Text is another (easy to implement) often forgotten tool to help search engines direct people to your site.
Title tags are very important for screen readers.
Hard to find Contact Information
Your email and phone number should be prominently displayed on every page.
Poor or Missing Content
The title says it all. This may be the most important of all and the hardest to implement. You have to hire a content writer - I believe it is worth every penny.
